Referring Clients to Inclusion ABA
 

Thank you for your interest in referring clients to Inclusion ABA! We appreciate your trust in our ability to provide high-quality, individualized ABA therapy services to children with autism spectrum disorder (ASD).
 

Who Can Make a Referral?

We welcome referrals from a variety of providers, including:

  • Pediatricians

  • Family Physicians

  • Psychologists

  • Psychiatrists

  • Speech-Language Pathologists

  • Occupational Therapists

  • Early Childhood Intervention Specialists

  • School Counselors

  • Social Workers

  • Other Healthcare Professionals
     

How to Make a Referral:

Making a referral is easy! You can:

  1. Download our Referral Form here

  2. Fax the completed form to: 936-570-9088

  3. Call us: 936-441-2200 to speak with our Intake Coordinator.

  4. Email our Provider Relations rep, Amy Borel-Witt at awitt@inclusionaba.com. Please note that email communication may not be HIPAA-compliant. To ensure the privacy of client information, we recommend using our secure referral form or contacting us by phone or fax. You are welcome to ask general questions via email.

  5. For families with questions about ABA therapy, have them contact our Intake Coordinator, Whitley Ellis at wellis@inclusionaba.com or call 936-441-2200.
     

Information to Include in the Referral:

To ensure a smooth transition for the client, please include the following information in your referral:

  • Client's full name and date of birth

  • Parent/guardian contact information

  • Diagnosis (including date of diagnosis)

  • Autism Diagnostic Report (most recent)

  • Reason for referral

  • Any relevant medical or developmental history

  • Current interventions or therapies

  • Insurance information
